Position Details:

• Title: RN Care Manager (Full-Time, W2)
• Location: Must reside within 15–20 miles of Boston, East Boston, Chelsea, Revere, Watertown, Roslindale, Roxbury, Dorchester, Hyde Park, Everett
• Work Setup: Hybrid – home visits + remote care coordination (Via Video and Phone calls)
• Salary: $90K–$105K (DOE)
• Sign-On Bonus: $5,000
• Schedule: Weekdays only (no weekends/on-call)
• Benefits: Full benefits + mileage reimbursement
• Tools Provided: Tablet & care documentation software
• Training: Paid 4-week onboarding + shadowing

Qualifications:

• Active RN license in Massachusetts
• 2+ years in LTC, Home Health, Hospice, or Elder Care
• Valid Driver’s License and willingness to travel locally
• Tech-savvy and self-motivated
• Bilingual Spanish a plus

The role focuses on supporting caregivers, not direct patient care. Interviews are being scheduled within 24–48 hours, and onboarding will start shortly.
